All cancellations are required to be made 48 hours in advance. When a patient cancels in less than this time, the appointment will be considered to have been missed. Appointments where the patient shows up more than 20 minutes late and cannot be seen, or are unwilling to wait to be seen at the next available same-day time, will also be counted as missed appointments.

When a patient misses two sequential appointments, they will be discharged from care at UGofSC (at the discretion of the provider) and referred out to another provider.

Missed Surgery Policy

We are taking COVID 19 precautions and although we try our best to have your appointment our within 30 minutes, there might be extended wait times. We will inform you if there is a delay due to unforeseen circumstances.  At that point you may reschedule or continue to wait. If waiting is an issue we have telehealth appointments available for routine follow ups.  

Our Wilshire location there is garage parking onsite with entrance on Wilshire Blvd. for a cost of up to $12.00 for one hour or more.

In Glendale there is ample street parking and a garage parking with entrance on Laurel Street for a cost of up to $7.00 for one hour or more. 

Our Burbank patients can access free parking across West Magnolia Blvd.

Please contact your medical group or insurance company prior to going to your lab to verify if your lab rquests are covered.  Otherwise, you may contact the lab directly and request an out of pocket cost prior to services rendered. 

We accept patients on the Medicare plan. As a courtesy, we will also bill your secondary insurance provider directly. You will be asked to sign an Advanced Beneficiary Notice for services not covered under Medicare. You shall be accountable for covering these services.

We accept patients on Medi-Cal. However, if you are enrolled with a medical group or IPA please check our list of medical groups that we accept under FAQ "Which Insurances do you accept?"

No, we don't provide this service. It is not covered by most insurance plans. We are happy to provide some basic testing that is needed prior to treatment, provided that it is a covered benefit under your insurance plan, however we don't have the appropriate expertise to provide any further service in this area.
